The mission of OpenPlacement is to make care transitions easy and to become the primary discharge planning tool of hospitals and Skilled Nursing Facilities in America. OpenPlacement engages patients early in the care transition process ensuring patient choice and reducing administrative work for case managers which equates to time and cost savings. Today, when a patient is being discharged from a hospital or skilled nursing facility (SNF), the discharge planner and/or family rely primarily on a stack of brochures, old printed “lists” and hours of phone calls in order to locate care providers that fit the particular needs of the patient. OpenPlacement is a real-time information system, which gives patients and families the ability to search in any geography, filter by various needs and availability and contact directly with post-acute care which significantly enhances and streamlines the discharge process.
